New Patient Registrations

We are currently accepting New patients.

Please either call into the surgery or telephone our enquiries number on 01296 423797. A receptionist will advise you on the documents (see below) and paperwork that have to be completed prior to registration. If your medical need is urgent the doctors can see you prior to registration, but you must officially register with the practice following this consultation.

For all patients wishing to register with the practice we will need to see one of the following original documents to confirm identity:

  • A current passport – all parts of the passport will be photocopied
  • Entry clearance documents – (stamped)
  • UK photocard driving licence with paper copy
  • Current residence permit (stamped)
  • Benefit/pensions book (not card)
  • Official tax notification
  • ID card for foreign nationals

You will also be asked to provide 2 of the following original documents to confirm residency:

  • Housing contracts
  • Utility bills – no older than 3 months (not mobile phone bills)
  • Bank statements
  • Residence permit
  • Official tax (or similar) document
  • Work permit

Training Practice

OAKFIELD SURGERY IS A TRAINING PRACTICE

This means doctors experienced in hospital medicine
(GP Registrars) join us for a year before embarking on careers in general practice.

They work under the supervision of Dr. Zaib and Dr. Chakraborty who are GP trainers.

Should you need urgent medial assistance whilst the surgery is closed please call NHS 111  

FedBucks is the name of your local out-of-hours service provider. The Out of Hours doctor is always available for emergencies, including nights, weekends and all Bank Holidays.  A nurse or doctor will assess whether you need either telephone advice, an appointment or a home visit if there is an urgent need.

We want to ensure that our patients are aware of what to do if they are ill or require medication when their GP Practice is closed.

Staff Training… Surgery Closed

The Practice will be closed from 1pm on the following dates for staff training. Should you need urgent medical assistance between 1pm and 8am the following morning call 111.
